Default Drivetrain Issues - Need help

Ok so I need help. I have a boosted 2000 Integra. Two weeks ago, I went out in the morning to go to work, and the clutch pedal was total mush. It went to the floor with no resistance. I looked at the master clutch cylinder, and found it was leaking into the vehicle interior, so I replaced it, bled the lines and put new fluid in. I drove the car for a few days and all seemed ok. Day 2 on the way home from work (approx 20 min in to a 30 min drive, ie, car was warm) on the freeway, in 5th gear, i tried to pass someone, gave it a little gas, and rpms sped up, but car did not accelerate (felt like clutch slipping). Re-bled lines, adjusted master cylinder(where it meets pedal), seemed ok again. Next day, on way home from work, same problem, only this time before i got home, i stopped at a stop sign right before my house, and with car in gear, foot all the way off of clutch pedal, car would not move. It was as if clutch was not engaging when foot off of pedal completely. Took it to local shop, they replaced clutch, flywheel, and re-bled the lines again. Said all was good, however on the way home from shop (again once car was warmed up) problem came back....

did they check/replace the slave cylinder?
