Re: which mounts?
You honestly need to search more. This topic is covered extensively on many different sites.
If your EG is a 5 speed then:
The upper tranny mount can be reused from the single cam.
The rear mount needs to come from a hydro b-series except the 6th gen civics (96-00). They use a different b-series mount.
The front torque mounts need to come from any hydro b-series, but the rubber mount can be reused.
The drivers side mount could be reused as well if the bracket on the LS is a two bolt and you have the corresponding two bolt mount. There is another three bolt type, but you need the three bolt bracket and the corresponding three bolt mount.
If your EG is an auto and you are converting to manual then for the upper tranny mount, you will need the custom mount from hasport or chop off the brackets and weld in the 5 speed brackets.

Re: which mounts?
Read my post a little better. I mentioned that the LS has a 2 bolt driver's side mount/bracket. That is the bracket on the block between the timing belt. If your civic came with the 2 bolt mount (most likely since I think only the EG Si's came witht the 3 bolt), then you can reuse the 2 bolt mount and not have to change the bracket on the block.
I would recommend filling the driver's side mount (2 bolt) since they suck compared to the 3 bolt one's.
