Default Replacing the clutch.... and or tranny?

Like I have said in other forums.. i think my clutch is slipping... it jerks through out gears. I have tried pulling the e brake and puting it in 3rd gear and slowly letting the clutch out.. it dies.. but not until the clutch pedal is almost completely out. Does that mean it is the beginning stages of my clutch starting to slip? Also, what exactly would i have to replace? The flywheel? pressureplates? And how would i know if they are bad.. would they just be all cracked and obvious? Could this by a problem with my tranny instead? I need any input or advice that could help me. Thanks
