Default Adapter plate questions

Those of you who are using adapter plates to mount your turbo, how did you secure the plate to the manifold? I have an 89 gt turbo probe an am going from the IHI to a t3 an can't for the life of me get my adapter plate to stay tight to my manifold. If anyone can be of some help with ideas, I'd greatly apprecate it, as of right now I am having a new plate made as I warped the last one due to it coming loose on one side. Makes for one hell of an exhaust leak...

Currently the adapter plate is bolted up to the manifold using counter sunk holes an 4 allen bolts torqued to about 50lbs

Are the holes in your plate threaded? Are you using a gasket between it and the mani?

I'm using a gasket between the mani and the flange. No, the holes that go through the plate to the mani are not threaded. Should I have that done in the next plate I have made?
