Commercial equipment might not need the neutral, but as far as residential is concerned the neutral is mandatory now. More of a safety thing I guess, even if the welder doesn't technicly require it.

It all depends on the welder. Older welder models only have provisions for 2 commons and 1 ground, but alot of newer 220v welders now have the neutral added in. If the welder doesn't have the neutral, you're not required to provide it. If it does, then you have no choice. It's kind of like electric dryers...older models create the neutral internally, newer ones have it seperated. Wish I had my NEC book in front of me, not sure of the newer revisions though as I'm limited to '05 and there's no reason for me to spend the money on a new one anymore.
