I've got a Centerforce Dual Friction Clutch. Clutch plate and pressure plate are what's included. Its used, but barely. I wouldn't say more than about 500-700 miles on it TOPS. Car it was in was totalled before it could be really used. I'm asking $240 + shipping OBO. New these things are about $340, so I'd say you'd be getting a pretty damn good deal.

I haven't ever had to tell anyone, but I like to use the 3 bumps and your out rule. I guess the upside is that you are the first.
