Need SMC Plus wiring instructions asap!

I have a SMC I need to wire up for a friend but he does not have the instructions any more. What wires go to what? Thanks in advance!

No instructions on his site. Anyone with an smc

Its only six wires and power and ground are obvious? Please Please Please!!!
