I also vote for out of time. or a bad valves. Just replaced the head on my B18B due to a bad valve. I would post a pic of the old head but dont know how.

sorry i didnt clerify .. new valves and seats new rings, pistons, rods, full rebuild.build .. gotta be cam timing

Cam timing probably is off a bit since you had the head milled!
