I actually encountered the same thing last night. Out of the blue a guy starts calling me about a car I tuned for him a long time ago saying somehow my tune went bad. I tell him that it doesn't work like that and something on his car must have failed I figured a map sensor or the tps or something.
I get over to his house to check it out for him I load his tune into the ostrich go drive around watch the readouts on the datalogger and nothing goes wrong car runs fine so I burnt him a new chip and we drove it again for about ten minutes and their was no problem what so ever. if I put the old chip back in it would go into limpmode as soon as it hit vtec? Ive seen a chip die in my own car as well.

When you burn the chip it electrically fuses links that represent binary values. It's possible, but not terribly common, to have those links become corroded/damaged over time.
